The 1/37 result is verified external-outcome evidence under the audit's strict closure standard, not a claim that only one item was completed.

A consequential fact could be captured in one place, contradicted in another, and never reach the person who needed to act.
Capture ≠ completion. Publication ≠ proof.Finding: individual capture and preservation mechanisms often worked; cross-lane reconciliation and verified closure were not dependable enough. All private examples are intentionally omitted.

I state the outcome. AI finds the context, catches contradictions, owns the follow-through, and returns proof—not another system for me to manage.
Every AI system needs a finisher.
Outcome stated → one existing task record → proof returned. Zero new queues.
